Register for Braille, Large Print and/or Audio Materials

To register you can mail or e-mail our one page application. Please note - if you already receive services from your state library for the blind and print disabled, the NLS or BARD, or if you are a member of the ACB or NFB, you can forgo filling out the application by calling us at 212-473-7800 or 800-637-9193. We can get you registered over the phone quickly and easily.

If you are not a patron of the NLS, it is still really easy to sign up via our application. On the back of the application there is a certification section which must be completed by any of the following: a medical professional, teacher of the visually impaired (TVI), librarian, your parish priest or Diocescan representative, or a professional staff member of a hospital, institution, or social welfare agency (e.g. Blinded Veterans Association, etc.). If you have a form from your doctor verifying your disability, that can be submitted with the application instead of a signature. We try to make it as seamless as possible to register. The form can be filled out online, printed, certified and scanned back to us at ClientServices@xaviersocietyfortheblind.org. It may also be snail-mailed back to us. Once we receive the application, we will contact you to let you know that you have become a patron.
